I grabbed tonight's nightly, installed the 0.9.0.28 Googlebar into the profile I already had... (ignoring my own advice!, so I can try and duplicate the average user experiences) and the save 50 search items remains checked after multiple browser restarts. I don't doubt you are having some problem... it is probably going to require intense scrutiny to discover the true cause. I did uninstall (and delete all files when win 2k asked) the browser before installing the nightly. I used both methods to call Googlebar options... the Tools pulldown menu, and the dropdown menu containing Googlebar Options... but the 50 items element remains selected. I have experience with other users and Firefox problems with disappearing bookmarks though... so I don't doubt your problem. 
Again try: 
"C:\Program Files\Mozilla Firefox\Firefox.exe" -P 
in a new Firefox shortcut. Create a new profile... and try that. If it works... you have something corrupting your old profile. Any corruption in prefs.js pretty much kills the user settings... that come after the corrupt line in the prefs.js file. Adblock killed my prefs.js a ways back. Everything after "A" in prefs.js was silently ignored. Thanks for the warning msg guys! :)

The other major thing is... did you uninstall the Googlebar before installing a higher version? If you did not do that... create a new profile. You can copy bookmarks.html from your old one, over the new one.
If that don't get you going you file a bug...
